For your convenience, we make our registration forms available online so that you can complete them and bring them in to your appointment. This helps to eliminate time spent in our waiting room.
When you come for your appointment, please remember to bring your insurance card (if applicable), your co-payment or deductible and a valid form of ID (such as a driver’s license).
The staff will schedule you with the earliest available appointment at a time most convenient for you. All patients must have the following upon arrival for appointment in order to be seen for pain management: Valid TN Driver’s License, 3 month Pharmacy History, Current Imaging Study Report (i.e. CT or MRI), Referral from PCP.
We accept Blue Cross Blue Shield and United Healthcare. Please bring your insurance card to your initial appointment, if applicable. We will assist you in filing your insurance claims; however, you are responsible for the unpaid balance or charges that your insurance does not cover.
All co-payments, deductibles and non-covered services are required to be paid at the time of service. For your convenience, we do accept checks, debit and credit cards.
When requesting refills, please be prepared to provide us with the name of the medication, the dosage and the phone number for your pharmacy. Please allow us until the end of our business day to get refills processed.
As a general rule, we do not process refill requests after hours or on the weekends unless it is an emergency.
